Intake Form & Consent Form

If you are a new client at Yvonne Dion Massage Therapy, you will be required to fill out an initial Intake/History form before your first appointment. This form provides detailed information about the client’s demographics, health, wellness, major concerns, etc. The detailed form helps me develop a good treatment plan for you. Each client is also required to read and sign a consent form, which allows me to provide you with massage treatments. By law, I cannot treat clients without this document being fully understood by the client and then signed. These two documents may be sent to the client through a secure server via the Jane app.

COVID-19 Protocol Information

COVID-19 information:

  • Hand sanitizer I have a government-approved hand sanitizer that each client will use when they arrive, and optionally before they leave.

  • COVID-19 client questionnaire Five hours before each appointment, my client will receive a COVID-19 email that must be filled out and emailed back to me prior to arriving.

  • Short notice appt cancellation or appt date change I will waive short notice cancellation or short notice appt change fees for any client that begins to have COVID-19 symptoms shortly before their treatment appt time, providing they fill out the Covid-19 survey questionnaire, showing their symptoms, and contact me right away by text only to make me aware this is the reason for their short notice cancellation or short notice appt change.

  • In my client bathroom, I offer a touchless soap dispenser and I will only offer disposable hand towels for client use at this time.

  • Therapist PPE I will wear a medical-grade mask during treatment.

  • Therapist hand/arm cleaning As per normal protocol, I will practice thorough hand/arm cleaning before and after each client treatment.

  • Extended time between each client bookings I will extend the time between bookings to allow for a more thorough cleaning as well as break time for myself.

  • Social Distancing There should be no need to worry about this because I'm a home-based sole-proprietor business owner. But just to be on the safe side when each client arrives, they can text or PM me to let me know they've arrived.

General Guidelines for Appointments

I request you to kindly arrive 20 to 30 minutes before your first appointment for your health history interview and treatment discussion. Arriving just in time for your massage therapy treatment will reduce your treatment time significantly as I require time interviewing you and discussing the treatment plan before your first treatment.


Arrival time for all other appointments after the first one, will be 10 minutes before your scheduled appointment time and include the following:

  • A quick interview with the therapist

  • Time taken by you to undress

  • Redressing time

  • Home care information at the end of the appointment

Note: You will still receive your full treatment time

Tip

  • Arriving ahead of time may allow you a bit of extra treatment time. I might also take you in for treatment a bit early if time permits. Possible bonus!

Appointment arrival time

  • After your first appointment, for all other appointments, kindly arrive at least 10 minutes before your appointment.

Arriving late for the appointment

  • When you arrive late, you will most probably lose precious minutes of your treatment, as you will have to spend a considerable time undressing, redressing, providing information, etc. As a result, you will not get the total areas worked on according to the treatment you booked. However, your fee amount will still be the same as for the treatment you booked. I recommend you arrive early to gain maximum benefits.

Appointment cancellation

  • I require 24 hours’ notice to cancel a scheduled treatment appointment. An appointment cancellation of fewer than 24 hours will result in you (the client) being charged 100% of the amount of your scheduled treatment. This fee will be charged to the credit card that is filed with me.

Exceptions: Medical emergency only - provided that you or someone close to you contact me by phone immediately upon knowing about the situation. First-time clients will still be required to pay this fee by email money transfer, for which I have auto-deposit, to the following email: yvonnedion7@gmail.com

Short notice appointment change rule

  • I require 8 hours’ notice to change a scheduled treatment appointment. An appointment change of fewer than 8 hours will result in you (the client) being charged $80. This fee will be charged to your credit card filed with me.

Exceptions: Medical emergency only - provided that you or someone close to you contact me by phone immediately upon knowing about the situation. First-time clients will still be required to pay this fee by email money transfer, for which I have auto-deposit, to the following email: yvonnedion7@gmail.com

No show to an appointment

  • You will be charged an $80 fee of your missed appointment. This fee will be charged to the client’s credit card I have on file with you.

Exceptions: Medical emergency only - provided that you or someone close to you contact me by phone immediately upon knowing about the situation. First-time clients will still be required to pay this fee by email money transfer, for which I have auto-deposit, to the following email: yvonnedion7@gmail.com  

A credit card on file

  • I require a current and valid credit card number on file for every client. This credit card will be used to pay for any treatments that you wish to take, for all cancelled or changed treatment appointments, and for all "no show" appointments (your credit card details will be stored in a very secure electronic location).

Payment method options

  • To be paid (by cash, E-transfer, or credit card, or by direct billing for treatment & optional tip by cash, E-transfer or credit card) immediately after treatment and home care instructions are completed. Clients may also optionally pre-pay (for their treatment and optional tip) before their treatment if they pay via e-transfer or credit card.

  • Tips are kindly accepted, appreciated and completely optional.

  • Please park in the driveway only on the RIGHT SIDE.

Allergy information

  • Mine is a pet free home. I use hypoallergenic massage lubricant.

  • The client’s verbal permission is always taken before I use the essential oil diffuser in the massage room.
